Infective endocarditis superimposed on a massively calcified severely stenotic congenitally bicuspid aortic valve.

چکیده
We describe a 55-year-old man who presented with a stroke resulting from active infective endocarditis (IE) involving a heavily calcified bicuspid aortic valve. The case highlights the infrequency of IE involving a heavily calcified valve, the inability of the infection to penetrate the calcific deposits, and the ability of the infection to spread to the adjacent soft tissues, leading to ring abscess and its multiple complications.

INFECTIVE ENDOCARDITIS IN CHILDREN
A total of 14 cases of infective endocarditis (IE) in children aged 6 months to 10 years were seen from December 1987 to December 1992 at the pediatric unit of Ayatollah Taleghani Medical Center. The majority of patients (12 of 14) were between 5 and 10 years of age. Acyanotic congenital heart disease was known to preexist in 78.6% and rheumatic valvular heart disease in 21.4% of cases. Or...
متن کاملGemella morbillorum endocarditis in a patient with a bicuspid aortic valve.
Infective endocarditis caused by Gemella morbillorum is a very rare occurrence and only a few cases have been reported in the literature so far. We describe a case of G. morbillorum endocarditis in a 31-year-old Pakistani woman who had a congenitally bicuspid aortic valve. She had also undergone repair of a large aortic aneurysm 1 year before this presentation. She presented to our institution ...
متن کاملInfective endocarditis is a disease in which a microorganism colonizes a focus in the heart, producing fever, heart murmur, splenomegaly, embolic manifestations, and bacteremia or fungemia
EPIDEMIOLOGY Predisposing factors for infective endocarditis are cardiac abnormalities that disrupt the endocardium by means of a jet injury, as well as the presence of blood-borne microorganisms that colonize these abnormal surfaces. Congenitally bicuspid aortic valve is the most common predisposing lesion for endocarditis of the aortic valve. In addition, congenital abnormalities of the aorti...
متن کاملPresence of a congenitally bicuspid aortic valve among patients having combined mitral and aortic valve replacement.
Although bicuspid aortic valve occurs in an estimated 1% of adults and mitral valve prolapse in an estimated 5% of adults, occurrence of the 2 in the same patient is infrequent. During examination of operatively excised aortic and mitral valves because of dysfunction (stenosis and/or regurgitation), we encountered 16 patients who had congenitally bicuspid aortic valves associated with various t...
متن کاملDilatation of the ascending aorta in patients with congenitally bicuspid aortic valves
INTRODUCTION The cause of ascending aortic dilatation occurring in patients with congenitally bicuspid aortic valves was investigated. METHODS Flow patterns through human aortic roots with congenitally bicuspid aortic valves as well as through porcine constricted aortas were studied in a left heart simulator. Vibration was recorded as a measure of turbulence in the post-stenotic segment. Hist...
